Who Qualifies For Assistance?

Who qualifies for the Elderly Waiver in Ohio? Qualifications vary by waiver. For all waivers Individuals may not have countable assets valued at more than $1,500. Also, Monthly income must not exceed 300 percent of the Social Security Insurance benefit. Further requirements are outlined below.

Ohio's PASSPORT Medicaid waiver program Eligible participants must:
• Be age 60 or older;
• Need hands-on assistance with dressing, bathing, toileting, grooming, eating or mobility;
• Receive services for which the cost does not exceed 60 percent of the cost of nursing home care;
• Meet financial criteria for Medicaid eligibility ; and
• Have their physicians agree to a service plan.

Ohio's Assisted Living Waiver Program Eligible participants must:
• Be age 21 or older;
• Need hands-on assistance with dressing, bathing, toileting, grooming, eating or mobility;
• Meet the financial criteria for Medicaid eligibility ; and
• Be able to pay the state established monthly room and board payment (rent).

Choices Home Care
Eligible participants must: Be age 60 or older;
• Need hands-on assistance with dressing, bathing, toileting, grooming, eating or mobility;
• Receive services for which the cost does not exceed 60 percent of the cost of nursing home care; and

Is There a Waiting List For Services?

How long is the waiting list for waiver services in Ohio? There are currently over 30,000 people waiting for waiver services in Ohio. There is greater need than resources available. County boards maintain waiting lists for services that are not available due to limited resources. This includes waiting lists for the IO Waiver, SELF Waiver, Level I Waiver, and possibly for locally funded services. In order to determine if an individual should be on a waiting list and the various options that may meet his/her needs, contact the your local county board of developmental disabilities.

What Services Are Offered & What Are The Service Limitations?

What services does the Ohio Medicaid waiver program offer? Each waiver provides different services.

Ohio's PASSPORT Medicaid waiver program Services available include: personal care, home delivered meals, adult day care, homemaker, emergency response, medical equipment and supplies, chore, social work counseling, nutritional counseling, independent living assistance and medical transportation. Participants also receive a Medicaid card that entitles them to Medicaid benefits including: physician care, hospitalization, prescription drugs and many other services. Case managers help participants develop a care plan and will monitor and adjust the plan according to the needs of the participant.

Ohio's Assisted Living Waiver Program Assisted Living Services include: 24 hour on-site response, personal care, supportive services (housekeeping, laundry, and maintenance), nursing, and transportation, 3 meals per day and social/recreational programming. Community Transition Services are available to individuals leaving a nursing facility to enroll in the Assisted Living waiver and helps them obtain essential household furnishings and other items.

Choices Home Care
Participants may receive the following services: adult day care, home care attendant service, home medical equipment and supplies, emergency response systems, minor home modifications, meals and pest control. A case manager will assist each participant in developing a care plan. The case manager will then monitor and adjust the plan according to the needs of the participant.

How Do You Select A Provider?

Who will provide my services?
- Assisted Living Medicaid Waiver Providers must be: Licensed as a residential care facility by the Ohio Department of Health; and Certified by the Ohio Department of Aging as an Assisted Living Provider.
-Choices Medicaid Waiver Providers are selected and hired by the program participants. Participants receive extensive training on serving as “employer of record” for the care providers they choose.
