California Assisted Living Waiver (ALW) - The waiver offers Assisted Care Services in three settings: Residential Care Facilities for the Elderly, Adult Residential Facilities and Public ally Subsidized Housing as well as Care Coordination, Residential Habilitation, and Nursing Facility Transition services. Qualified participants have full-scope Medi-Cal eligibility with zero share of cost and are determined to meet the Skilled Nursing Facility Level of Care, A or B. The ALW is currently implemented in the following counties: Alameda, Contra Costa, Fresno, Kern, Los Angeles, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Joaquin, San Mateo, Santa Clara and Sonoma Counties. Additional Counties may be added in the future. There is no Waitlist.
Nursing Facility / Acute Hospital (NF/AH) - This waiver offers services in the home to Medi-Cal beneficiaries with a long-term medical condition for who, in the absence of this waiver, would otherwise receive care for at least 90 days in an intermediate care facility, a skilled nursing facility, a subacute facility, or an acute care hospital. Services include: Shift Nursing; Home Health Aide; Waiver Personal Care Service;s Transitional Case Management; Case Management; Home Care Training; Emergency Response Systems; Home Modifications; Respite Care; Utility Reimbursement; and Community Transition. There is a wait list for individuals assessed at the intermediate care facility and skilled nursing facility level of care unless those individuals are currently residing in one of those facilities. Family members may provide Waiver Personal Care Services.
