Who Qualifies For Assistance?

What is considered a developmental disability in Colorado? The individual must meet financial and program criteria to access services under the Waiver for Persons who are Elderly, Blind, and Disabled (HCBS-EBD program). To utilize waiver benefits, individuals must be willing to receive services in their home or community. The following criteria must be met to qualify:

  • Level of Care - Individuals require long term support services at a level comparable to services typically provided in a nursing facility.
  • Eligibility Group - Individuals must be 18 years or older - Individuals between ages 18 and 64 must be blind or have a physical disability - Individuals over 65 must have been determined to have a significant functional impairment
  • Financial - An individual’s income must be less than $2,094.00 per month. - For a single person, their countable resources must be less than $2,000.00 and for a couple it must be less than $3,000.00.

To become a PASA an application for Program Approval must be submitted to DDD and a Medicaid Provider Agreement must be submitted to HCPF to enroll as a Medicaid Provider. There is no fee to become a provider.

  • WAIVER for PERSONS DEVELOPMENTALLY DISABLED: For persons with developmental disabilities age 18 and older. The person receiving services cannot be living with his or her family.
  • SUPPORTED LIVING SERVICES WAIVER: To provide to persons with developmental disabilities supported living in the home or community. These Medicaid waiver funded support services are for adults who either can live independently with limited supports or who, if they need extensive support, are getting that support from other sources, such as their family. For ages 18 and older.
  • CHILDREN WITH AUTISM WAIVER: For children with a medical diagnosis of Autism who are most in need due to the severity of their disability. Children must meet additional targeted criteria; Birth through age 5. This waiver is handled by Colorado's Health Care Policy and Financing Agency.
  • CHILDREN'S EXTENSIVE SUPPORT WAIVER: for children with developmental disabilities or delays, that are most in need due to the severity of their disability. This program serves children (birth through age 17) who have significant medical and/or behavioral needs. These children are at high risk of out-of-home placement and require near constant line-of-sight supervision due to one or more of the following: (a) self-injurious or self-endangering behavior or medical condition which, without intervention, will result in a life threatening condition or situation; (b) a demonstrated pattern of serious aggression toward self, others or property; or (c) constant vocalizations which are a significant disruption to family life.
  • CHILDREN'S HCBS WAIVER: For disabled children, birth through age 17, who would otherwise be ineligible for Medicaid due to excess parental income and/or resources. Children must be at risk of nursing facility or hospital placement.
  • CHILDREN'S HABILITATION RESIDENTIAL PROGRAM WAIVER: Provides residential services for children and youth in foster care who have a developmental disability and extraordinary needs. Children must be at risk for institutionalization. For birth to 21 years of age.
  • HCBS WAIVER for PERSONS with BRAIN INJURY: To provide a home or community based alternative to hospital or specialized nursing facility care for persons with brain injury. For ages 16 through 64.
  • HCBS WAIVER for PERSONS with MENTAL ILLNESS: To provide a home or community based alternative to nursing facility care for persons with major mental illness. For ages 18 and older.
  • HCBS WAIVER for PERSONS LIVING WITH AIDS: To provide a home or community based alternative to hospital or nursing facility care for persons living with HIV/AIDS. For all ages.
  • HCBS WAIVER for PERSONS who are ELDERLY, BLIND, AND DISABLED: To provide a home or community based alternative to nursing facility care for elderly, blind, and disabled persons. For ages 18 and older.
  • Pediatric Hospice Waiver (HOPEFUL Program): This waiver provides access to Medicaid services for children with a life-limiting illness. The HCBS Pediatric Hospice Waiver (HCBS PHW) is for children from birth to age nineteen with a medical diagnosis of a life-limiting illness who meet the institutional level of care for inpatient hospitalization. The HCBS PHW allows children to live in their community while keeping them out of institutions.
